The analysis of dried blood spot samples (DBS) by liquid chromatography – high resolution mass spectrometry (LC-HRMS) for assessing adherence to candidate cardiovascular therapeutic drugs was investigated. To evaluate the method 8 mm discs were punched from each DBS and extracted followed by subjecting to LC-HRMS analysis. Trials using the top 11 UK prescribed cardiovascular drugs are reported demonstrating the ability of the system to detect the target analytes during the 24 hour repeat prescription cycle. The system responded successfully to challenges by samples from volunteers of known adherence or receiving no medication. Examples of non-adherence to different medications were identified.en
